What is the relationship between electric scalpels and thermofusion?

Electrosurgery is defined as the medical procedure performed using electrical current to cut, remove, dispose of malignant tissue and keep blood vessel bleeding under control. This surgical technique consists of using current through a special electrode that is placed on the tissue or the part that requires it to perform the surgical process properly.

Electrosurgery is nowadays the most commonly used surgical procedure in operating rooms. In virtually every operating room in the world there is an electrosurgical equipment, as they are applicable to all surgical specialties, both in hospitals and clinics and in medical consultations. Two of the great equipment used in this discipline are the electrical scalpel and thermofusion clamps, closely related equipment, since both use the electric energy, each with particular purpose, thus achieving a specific effect.

What is an electric scalpel?

An electric scalpel or electroscalpel is a medical equipment that allows the use of high frequency alternating energy to achieve a desired effect (cutting, coagulating or cauterizing) in a surgical procedure, this is possible thanks to the ability of the equipment to modulate the current.

This equipment has the ability to transform electrical energy into heat to cut or coagulate tissues and cauterize them at the same time, using currents that develop at frequencies above 200,000 Hz.

How does an electric scalpel work?

To operate an electric scalpel uses high-frequency electrical energy (>200,000 Hz), to produce heat, with the aim of coagulating or cutting tissues of biological origin. Depending on its structure, it has two modes of operation. The monopolar mode, where the active electrode is the one that intervenes in the process, since it concentrates the energy at the contact point. And the bipolar modality that involves the action of the two electrodes, and they usually come in the form of a hemostatic clamp.

Both modalities perform pure and combined electrosection, as well as electrocoagulation and heat production is achieved through the passage of an oscillatory electric current, concentrated in a small area. This device uses isolated or closed systems, that is, the current passes through the patient’s body or a portion of tissue, and eventually returns to the device.

What is thermofusion?

Vascular thermofusion or sealing is the process that allows the sealing of blood vessels and tissue bundles, its operation is based on the fusion of collagen and elastin of the vascular walls and generating an autologous and permanent sealing.

Which allows that for blood vessels with a diameter less than 7 mm no staples, or sutures are needed, because thermofusion offers protection against possible bleeding.

How does a thermofusion clip work?

In order to generate the vascular thermofusion or sealing the tissue is held with the heat-generating clamp and thermofused with the appropriate current for this purpose, that is to apply heat at a certain intensity and time, so that the closing of the blood vessels and cutting of the tissues occurs without the need to emit electric shocks between them. Typically, thermofusion instruments have an automatic stop function that disconnects the application as soon as optimum thermofusion is achieved.

One of the great benefits of using this device is that thermofusion is limited to the instrument’s clamping area, minimizing lateral thermal injuries and protecting adjacent tissue. Their results are superior to those obtained with other equipment such as standard bipolar electroscalpel and ultrasonic coagulator, and similar to those provided by ligatures, clips and sutures. Using elastin and collagen from blood vessels, you get very good haemostasis at the level of arteries and veins, but it is worth noting that thermofusion loses effectiveness when applied to tissues where the blood supply is mainly based on capillaries (little collagen).
